Do you know who the doctor is?（その医者が誰だか知ってますか。）
Do you know who is a doctor?（誰が医者だか知ってますか。）
Do you know who is the doctor?（誰がその医者だか知っていますか。）
という感じかな。
追加で（テーマが違うので本当は追加しちゃいかんのだが）。
do you think と do you know の使い方の違い。
「ニック式英会話」より。
「do you think は文中に挿入されるが、Do you know は文頭に来る。」

Who do you think is a doctor?
Who do you think is the doctor?
Who do you think is coming to us?
Do you know who is coming to us?
（むろん、that 節をとって文頭に来る Do you think 等はある。）
